Facilitation Letter of Intent
A form letter executed by a church, religious community, or individual congregant formally requesting specified entheogenic facilitation services. This document should be used for purposes of establishing the religious intent of the parties and relative ceremonial quantity while transporting sacrament to a ceremony or ritual.
Participant Certification of Intent Form
A participant-endorsed form certifying their participation in the ritual or ceremony is not for recreational or therapeutic purposes. Further affirms the sacred intent behind the facilitation. This form also has participants certify that their particpation is insatisfaction of religious/spiritual motivations. This form also has participants certify their understanding that facilitation or guide services aren’t and are not intended to be for the purposes of diagnosing and/or treating any medical ailment, sickness, or condition. This form also informs participants of potential criminal liability under 21 U.S.C. § 843(a)(3) for “…acquiring or obtaining possession of a controlled substance by misrepresentation, fraud, forgery, deception, or subterfuge.”
